I am Kelly McNamara and I run Longevity Lounge Esthetics (ABN 75 781 157 040) on my own, at Shop 14-16 / 413-417 New South Head Road, Roma Arcade, Double Bay NSW 2028.
This page explains how I handle your personal information. I follow the Privacy Act 1988 (Cth), including the Australian Privacy Principles, and the NSW Health Records and Information Privacy Act 2002.
I collect most information from you: in person, including at consultation, and by phone, text, email or social media message.
With your agreement, I also receive information from your surgeon or medical team.
You can ask general questions without giving your name. I need your name and health history before I can treat you.
If you do not give me the information I need, I may not be able to treat you safely.
I take photos and skin scans to plan your treatment and track your progress. Healing photos you send me help me check your recovery. All of these go into your treatment file.
I do not put client photos on this website.
I only show your photos to anyone else, including other clients, if you agree in writing. You can withdraw your agreement at any time.
My skin scanner may produce automated readings or scores from images of your skin. I use these only to help me suggest treatments. I make every treatment decision myself, with you.
I do not sell or rent personal information. I only share it:
Some services I use, such as phone backup, email and messaging, may store information on servers outside Australia, mainly in the United States. Google may also receive website visitors' IP addresses in the United States and other countries.
I keep your information secure. Only I use your treatment file. The services listed above hold or handle some of it for me.
I keep health records for at least 7 years after your last treatment. If I collected them before you turned 18, I keep them until you turn 25. After that I destroy them securely.
If a data breach is likely to cause you serious harm, I will tell you and the Office of the Australian Information Commissioner.
I only send marketing messages, such as offers or a newsletter, if you have agreed to receive them. Every message lets you unsubscribe easily. I only use your health information for marketing if you have expressly agreed.
You can ask to see the information I hold about you, or ask me to correct it. Contact me by phone, text, email or in person.
I will respond within 30 days. There is no charge to ask, or to correct your information. If you want copies, I may charge a reasonable fee for my time, and I will tell you the amount first.
If I cannot give you access or make a correction, I will tell you why in writing and how to complain.
Please contact me first: 0400 477 855 or [email protected]. I will acknowledge your complaint within 10 business days and aim to resolve it within 30 days.
If you are not satisfied with my response, you can complain to either of these:
A complaint to the NSW Privacy Commissioner must be in writing and is usually made within 6 months of when you first became aware of the problem.
I update this policy whenever my practices change. The current version is always on this page.
Ask me if you would like to know when it last changed, or if you would like a free copy in another format.
